Engineering Services Exam, 2013: Online Detailed Application Form (DAF) for selected candidates

All the candidates qualified on the basis of the result of the written part of the Engineering Services Examination, 2013 are requested to fill the detailed application form (DAF) online.

B. Read the following instructions before filling up the online Detailed Application Form.
1. HOW TO APPLY:
a. Candidates must apply Online by using the website http://www.upsc.gov.in. The Online Detailed Application Form
can be filled from 13-11-2013 to 25-11-2013 till 11.59 PM after which the link will be disabled.
b. Candidates should ensure that all columns of the Online Detailed Application Form are filled in correctly. No
correspondence will be entertained by the Commission from candidates to change any of the entries made in the
Detailed Application Form.
2. LAST DATE FOR RECEIPT OF PRINTED COPY OF ONLINE DETAILED APPLICATION:
a. Candidates are required to take the print out of the Finally Submitted online Detailed Application Form and sign
on each page and submit it to the Commission along with certificates duly attested and Summary Sheet.
The last date for receiving printed copy of online Detailed Application along with enclosures/certificates and
Summary Sheet is 29-11-2013 (5:00 PM). The envelope containing the printed copy of the Online Detailed Application
Form should be superscribed with “Detailed Application for Engineering Services Examination, 2013” and must reach
the Under Secretary (E.VII), Union Public Service Commission, Dholpur House, Shahjahan Road, New Delhi – 110069 on
or before the date specified above.
b. Detailed Application Form can also be delivered at Union Public Service Commission counter by hand on or before
the date specified above. The Commission will not be responsible for the applications delivered to any other
functionary of the Commission.

Guidelines for filling the online Detailed Application Form (DAF) for Engineering Services Examination, 2013
www.upsc.gov.in and click on the link “Detailed Application Form for Engineering
Services Examination, 2013”.
2. Candidate who wish to apply for Engineering Services Examination, 2013 have to first get themselves registered
before filling up the Online Detailed Application Form(DAF) at the website www.upsc.gov.in
3. Please register yourself by clicking appropriately on the link provided below the “login” button.
4. On the registration module, provide your Name (as recorded in the matriculation/secondary examination
certificate), Roll Number (in six digit format) and Date of Birth (as recorded in the matriculation/secondary
examination certificate).
5. Provide your e-mail address and re-enter your e-mail address.
6. Enter the random number which is appearing on the screen and click the submit button.
NB: (a) All the field are compulsory to fill in.
(b) Please ensure that you provide a valid and an active e-mail address in the e-mail address field as your password
for filling up the application form shall be e-mailed to you on the e-mail address provided by you. Invalid or inactive
e-mail address may prevent you from smooth completion of submission of online Detailed Application Form.
7. After clicking the submission button, you will get a message on your screen, clearly mentioning your Name and email
that you have been registered and your password has been sent to your e-mail address. Please note down
your password.
8. Now log in to your e-mail account and note down your password which has been sent to you by the Commission
and again go back to the website (www.upsc.gov.in) and log in into the Login module with your Roll Number (six
digit number) along with the password.
9. Online DAF has seven modules as indicated above, namely- Personal, Educational Information, Parental
Information, Employment Information, service references and Final Submission.
10. Candidates are required to fill all the modules completely before finally submitting the form ONLINE, using the
Final Submission Module.
11. Candidates should save each module after completing the same. However, candidates will have the option to make
changes in any module before final submission of DAF. Once Final Submission has been made, then no change will
be possible Online.
12. Candidates are required to take the print out of the Finally Submitted online Detailed Application Form and sign on
each page and submit it to the Commission along with certificates duly attested. This application along with
documents/enclosures should be submitted to the commission positively by 29-11-2013 (5:00 PM).
13. Candidates are not supposed to print the PREVIEW OF APPLICATION. In case the candidates submit print copy of
PREVIEW OF APPLICATION, his candidature is liable to be rejected.
14. Candidates may note that some of the fields namely Candidate's name, Date of Birth, Father's name, Mother's
name etc. have been pre-populated using the information provided by the candidate at the time of filling up the
Common Application Form. Such fields will appear grayed out and cannot be changed by the candidate. However,
if a Candidate notes any error / discrepancy in these fields, he may make a separate detailed representation to the
Commission including all relevant documents, in support of his claim. The representation may be sent along with
the filled up hard copy of the Detailed Application Form. The Commission will decide each such case on merit and
its decision in such cases shall be final.

Instructions to Candidates for filling in the
ApplicationForm
1. The candidates should read these instructions
carefully before filling in the application form.
2. Candidates must use only International form of
Indian numerals in filling in the application form, e.g.
1, 2, 3, 4, 5, 6 etc. They should take special care that the
entries made in the application form should be clear and
legible. In case there are any illegible or misleading
entries, the candidates will be responsible for the
confusion and the ambiguity caused in interpreting such
entries. Corrections, if any, should be legible and attested
by the candidates.
3. Candidates are required to specify clearly in
Column 11 of the application form the Services/Posts for
which they wish to be considered in order of preference.
They are advised to indicate as many preferences as they
wish to, so that having regard to the rank in the order of
merit, due consideration can be given to their preferences
when making appointment.
N.B. (i)—Departmental candidates admitted to the
examination under age relaxation vide Rule 5(b) may give
their preferences for the Services/Posts in other Ministries/
Departments also.
N.B. (ii)—Candidates admitted to the examination
under the proviso to Rule 6 will be considered only for the
posts mentioned in the said proviso, and their preference
for other Services and Posts, if any, will be ignored.
N.B. (iii)—No request for addition/alteration in the
preferences indicated by a candidate in his/her application
will be entertained by the Commission.
N.B. (iv)—Candidates are also advised that the
hard copy of the DAF to be submitted by them should
preferably be printed on A4 size paper.
4. The information earlier given by the candidates
in the simplified application form for the Examination will
be cross-checked with the information given by them in
this application form. If there are any serious discrepancies,
their candidature will be cancelled.
5. The candidates should paste a recent passport size
photograph in the space provided for it in the application
form. The photograph should be duly attested by a Gazetted
Officer/Principal of the College/Institution last attended.
6. A candidate who claims to belong to one of
the Scheduled Castes, the Schedule Tribes or the
Other Backward Classes should submit in support
of his claim an at tested/cer t i fied copy of the
certificate in the form given in Appendix-I from
the District Officer or the Sub-Divisional Officer
or any other officer as indicated in Appendix-I of
the district in which his/her parents (or surviving parent)
ordinarily reside. Such an officer should have been designated
by the State Government concerned as competent to issue such a
certificate. If both his/her parents are dead, the officer
signing the certificate should be of the district in which the
candidate himself/herself ordinarily resides otherwise than
for the purpose of his/her own education.
7. Acandidate must submit alongwith his/her application
attested/certified copy of a certificate showing his/her
educational qualification prescribed in para 3(III) of the Notice
for the Examination published in the Employment News dated
6th April, 2013 and in Rule 6 of the Rules for the Examination
published in the Gazette of India dated 6th April, 2013. The
certificate submitted must be one issued by the authority (i.e.
University or other examining body) awarding the particular
qualification.
8. A candidate must enclose with his/her application an
attested/certified copy of certificate of age (indicating his/
her date of birth). The date of birth accepted by the Commission
is that entered in the Matriculation or Secondary School
Leaving Certificate or in a certificate recognised by an Indian
University as equivalent to Matriculation or in an extract from
a Register of Matriculates maintained by a University and that
extract must be certified by the proper authority of the
University. A candidate who has passed the Higher Secondary
Examination or an equivalent Examination may submit an
attested/certified copy of the Higher Secondary Examination
certificate or an equivalent certificate.
No other document relating to age like horoscopes,
affidavits, birth extracts from Municipal Corporation, Service
Records and the like, will be accepted.
The expression Matriculation/Higher Secondary
Examination certificate in this part of the instruction includes
the alternative certificate mentioned above.
Sometimes the Matriculation/Higher Secondary
Examination Certificate does not show the date of birth or
only shows the age by completed years or completed years
and months. In such cases a candidate must send in addition to
the Matriculation/Higher Secondary Examination Certificate
an attested/certified copy of the certificate from the
Headmaster/Principal of the Institution from where he/she
passed the Matriculation/Higher Secondary Examination
showing date of his/her birth or his/her exact age as recorded
in the Admission Register of the Institution.
Candidates are warned that unless complete proof
of age as laid down in these instructions is sent with the
application, the candidature of the candidate will be
rejected.
Note 1: A candidate who holds a completed Secondary School
Certificate need submit an attested/certified copy of
the page containing entries relating to age only.

Note 2 : Candidates should note that only the date of birth as
recorded in the Matriculation/Higher Secondary
Examination Certicate or an equivalent certificate
on the date of submission of application will be accepted
by the Commission and no subsequent request
for its change will be considered or granted.
Note 3 : Candidates should also note that once date of birth
has been claimed by themand entered in the records
of the Commission for the purpose of admission
to an examination, no change will be allowed
subsequently or at a subsequent examination on any
ground whatsoever.
9 (i) A candidate claiming age-relaxation as a
domicile of the State of Jammu and Kashmir should
produce an attested/certified copy of a certificate from the
District Magistrate in the State of Jammu and Kashmir
within whose jurisdiction he/she had ordinarily resided or
from any other authority designated in that behalf by the
Government of Jammu and Kashmir to the effect that he/
she had ordinarily been domiciled in the State of Jammu
and Kashmir during the period from the 1st January, 1980
to the 31st day of December, 1989.
(ii) Acandidate disabled while in the Defence Services
claiming age concession should produce an attested/certified
copy of the certificate in the form given in Appendix -II,
from the Director General Resettlement, Ministry of
Defence to show that he/she was disabled while in the
Defence Services in operations during hostilities with any
foreign country or in a disturbed area and released as a
consequence thereof.
(iii) Ex-servicemen including Commissioned Officers
and ECOs/SSCOs claiming age concession should produce
an attested/certified copy of the certificate as applicable to
them, in the form given inAppendix-III, from the authorities
concerned.
(iv) A Government Servant claiming age concession
under rule 5(b) should submit a certificate in Original from
the Head of the Department/Office in the format given in
Appendix-IV.
10. A candidate claiming to belong to physically
disabled category shall be required to meet one or more of
the physical requirement/abilities, as laid down in Rule 19
of the Rules for the Engineering Services Examination, 2013,
for performing the duties in the concerned Services/Posts.
He/She must submit a certified copy of the certificate issued
by Government Hospital/Medical Board in the format given
inAppendix-V.
11. The candidates should note that their candidature
for the Engineering Services Examination, 2013, will be purely
provisional, subject to their satisfying the prescribed eligibility
conditions. If on verification at any stage before or after the
examination it is found that they do not fulfil any of the
eligibility conditions, their candidature for the examination
will be cancelled by the Commission.
12. Enclosures to the application form to be sent to the
Commission by the candidates are as follows :—
(i) An attested/certified copy of the certificate of age.
(ii) An attested/certified copy of the certificate of
educational qualification indicating the subjects.
(iii) An attested/certified copy of the certificate in
support of claim to belong to Scheduled Castes/
Scheduled Tribes/Other Backward Classes
(wherever applicable).
(iv) Undertaking by the candidate seeking releaxation
as OBC candidate in support of claim of not
belonging to creamy layer, to be given in format as
mentioned in para B-1 ofAppendix-I.
(v) An attested/certified copy of certificate in support
of claim for age concession (wherever
applicable).
(vi) AnAttested/certified copy of certificate in support
of claimto being physically handicapped (wherever
applicable).
Originals of the above certificates are required to
be produced at the time of interview.
NOTE :—Candidates are required to sign the attested/
certified copies of all certificates sent along with the
application form and also to put the date.
13. Any application received without all or some of
the enclosures will entail cancellation of candidature. Any
missing enclosures sent subsequently by the candidate will
not be entertained. The candidates must ensure that the
application form is properly filled in and is accompanied by
all the relevant enclosures. No column of application should
be left blank.
14. In all communications with the Commission
regarding this application, the candidate should mention the
name of Examination, his full name, Roll Number and date
of birth.

उम् मीदवारों को आवेदन ूपऽ भरने के बारे में अनुदेश
1. उम् मीदवारों को चािहए िक वे आवेदन ूपऽ भरने से पहले इन
अनुदेशों को ध् यान से पढ़ लें ।
2. उम् मीदवार यह अवँ य ध् यान रखें िक उन् हें आवेदन-ूपऽ को भरने
में केवल भारतीय अंकों के अंतरार्ं शीय ः वरूप का ूयोग करना है
जैसे 1,2,3,4,5,6 इत् यािद । वे इस बारे में िवशेष सावधानी बरतें
िक आवेदन-ूपऽ में की गई ूिविं टयां ः पं ट और सुपाठ्य हों ।
यिद वे ूिविं टयां पढ़ी नहीं जा सकती हैं या ॅामक हैं तो उनके
िनवर्चन में होने वाली ॅांित या संिदग् धता के िलए उम् मीदवार ही
िजम् मेदार होंगे । संशोधन, यिद कोई हों, ः पं ट होने चािहएं तथा
उम् मीदवार द्वारा अनुूमािणत होने चािहएं ।
3. उम् मीदवारों से अनुरोध है िक वे आवेदन-ूपऽ के कालम 11 में उन
सेवाओं/पदों को ः पं ट रूप से िलख दें िजनके िलए वे वरीयता
बम में िवचारण चाहते हैं । उन् हें सलाह दी जाती है िक वे िजतनी
चाहें उतनी वरीयताओं का उल् लेख कर दें तािक योग् यताबम में
उनके रैंक को ध् यान में रखते हुए िनयुिक् त करते समय उनकी
वरीयताओं पर भली-भांित िवचार िकया जा सके ।
िवशेष ध् यान (i) :- वे िवभागीय उम् मीदवार, िजन् हें आयु सीमा में छूट के
अधीन [देिखए िनयम 5 (ख)] परीक्षा में भाग लेने की अनुमित दी गई
है अन् य मंऽालयों/िवभागों में सेवाओं/पदों पर िनयुक् त िकए जाने के
िलए भी अपनी वरीयता दे सकते हैं ।
िवशेष ध् यान (ii) :- िनयम 6 के उपबंध के अंतगर्त परीक्षा में ूवेश
िदए गए उम् मीदवारों की केवल उन् हीं वरीयताओं पर िवचार िकया जाएगा
जो उक् त उपबंध में िनिदर्ं ट पदों के िलए हैं और अन् य सेवाओं और पदों
के िलए उनकी वरीयताओं, यिद कोई हों पर िवचार िकया नहीं जाएगा ।
िवशेष ध् यान (iii) :- िकसी भी उम् मीदवार का अपने िवः तृत आवेदन-
ूपऽ में पहले से िनिदर्ं ट वरीयताओं को बढ़ाने/पिरवतर्न करने के बारे
में कोई अनुरोध आयोग द्वारा ः वीकार नहीं िकया जाएगा ।
िवशेष ध् यान (iv) :- उम् मीदवारों को यह सलाह भी दी जाती है िक उनके
द्वारा जमा िकए जाने वाला िवः तृत आवेदन पऽ ए-4 आकार के कागज
पर मुिित हो।
4. उम् मीदवार द्वारा इस आवेदन-ूपऽ में दी गई सूचना की पड़ताल
इस परीक्षा के सरलीकृत आवेदन-पपऽ में दी गई सूचनाओं से की
जाएगी । गंभीर िवसंगितयां पाई जाने पर उनकी उम् मीदवारी रद्द
कर दी जाएगी ।
5. उम् मीदवार आवेदन-ूपऽ में िदए गए ः थान पर ठीक ूकार से
अपना हाल ही का पासपोटर् आकार का एक फोटो िचपकाएं । फोटो
िकसी राजपिऽत अिधकारी अथवा उस महािवद्यालय/संः थान जहां
उम् मीदवार सबसे बाद में अध् ययनरत रहा हो के ूाचायर्, द्वारा
िविधवत ् सत् यािपत हो ।
6. यिद कोई उम् मीदवार िकसी अनुसूिचत जाित/अनुसूिचत जनजाित
या अन् य िपछड़ी ौेिणयों का होने का दावा करते हैं तो उसे अपने
दावे के समथर्न में उस िजले के िजसमें उसके माता-िपता (या
जीिवत माता या िपता) आमतौर से रहते हों, िजला अिधकारी या
उप-मंडल अिधकारी या पिरिशं ट-I में दशार्ए गए ऐसे अन् य
अिधकारी से िजसे संबद्ध राज् य सरकार में यह ूमाण-पऽ जारी
करने के िलए पद नािमत िकया हो पिरिशं ट-I में िदए गए फामर्
में ूमाण-पऽ लेकर उसकी अनुूमािणत/ूमािणत ूितिलिप ूः तुत
करनी चािहए । यिद उम्म ीदवार के माता और िपता दोनों की मत्ृ य ु
हो गई हो तो यह ूमाण-पऽ उस िजले के अिधकारी से िलया
जाना चािहए जहां उम् मीदवार अपनी िशक्षा से िभन् न िकसी अन् य
ूयोजन से आमतौर पर रहता है ।
7. उम् मीदवार को इस ूपऽ के साथ ऐसे ूमाण-पऽ की
अनुूमािणत/ूमािणत ूितिलिप अवँ य भेजनी चािहए िजससे इस
बात का ूमाण िमल सके िक “रोजगार समाचार” िदनांक 6 अूैल,
2013 में ूकािशत उक् त परीक्षा के नोिटस के पैरा 3 (।।।) में और
भारत के राजपऽ िदनांक 6 अूैल, 2013 में ूकािशत उक् त परीक्षा
की िनयमावली के िनयम 6 में ूकािशत िनधार्िरत शैिक्षक योग् यता
उसके पास है । भेजा गया ूमाण-पऽ उस अिधकारी (अथातर् ्
िवँ विवद्यालय या अन् य िकसी परीक्षा िनकाय) का होना चािहए
िजसने उसे योग् यता िवशेष ूदान की है ।
8. उम् मीदवार को आयु ूमाण-पऽ िक िजसमें जन् म की तारीख बताई
गई हो, अनुूमािणत/ूमािणत ूित अपने आवेदन के साथ भेजनी
चािहए । आयोग जन् म की वह तारीख ः वीकार करता है जो
मैिशकुलेशन या माध् यिमक िवद्यालय छोड़ने के ूमाण-पऽ या िकसी
भारतीय िवँ विवद्यालय द्वारा मैिशकुलेशन के समकक्ष माने गए
ूमाण-पऽ या िकसी िवँ विवद्यालय द्वारा अनुरिक्षत मैिशकुलेशन के
रिजः टर में दजर् की गई हो और वह उद्धरण िवँ विवद्यालय के
समुिचत ूािधकारी द्वारा ूमािणत हो । जो उम् मीदवार उच् चतर
माध् यिमक परीक्षा या उसकी समकक्ष परीक्षा उत् तीणर् कर चुका है
वह उच् चतर माध् यिमक परीक्षा या समकक्ष परीक्षा के ूमाण-पऽ
की अनुूमािणत/ूमािणत ूितिलिप ूः तुत कर सकता है ।
आयु के संबंध में कोई अन् य दः तावेज जैसे जन् म कुण् डली,
शपथ-पऽ, नगर िनगम से तथा सेवा अिभलेख से जन् म संबंधी
उद्धरण तथा ऐसे ही अन् य ूमाण-पऽ ः वीकार नहीं िकए जाएंगे ।
अनुदेशों के इस भाग में आए “मैिशकुलेशन/उच् चतर
माध् यिमक परीक्षा ूमाण-पऽ” वाक् यांश के अंतगर्त उपयुर्क् त
वैकिल् पक ूमाण-पऽ सिम् मिलत है ।
कभी-कभी मैिशकुलेशन/उच्चतर माध्यिमक परीक्षा ूमाण-पऽ में
जन्म की तारीख नहीं होती है या आयु के केवल पूरे वषर् या/और महीने
ही िदए जाते हैं, ऐसे मामलों में उम्मीदवारों को मैिशकुलेशन/उच्चतर
माध्यिमक परीक्षा ूमाण-पऽ के अितिरक्त उस संःथा के
हैडमाःटर/िूंिसपल से िलए गए ूमाण पऽ की अनुूमािणत/ूमािणत
ूितिलिप भेजनी चािहए जहां से उसने मैिशकुलेशन/उच्चतर माध्यिमक
परीक्षा उत्तीणर् की हो । इस ूमाण-पऽ में उस संःथा के दािखला
रिजःटर में दजर् की गई उसकी जन्म की तारीख या वाःतिवक आयु
िलखी होनी चािहए ।
उम्मीदवारों को चेतावनी दी जाती है िक यिद आवेदन-ूपऽ के साथ
इन अनुदेशों में यथा िनधार्िरत आयु का पूरा ूमाण नहीं भेजा गया तो
उम्मीदवार की उम्मीदवारी रद्द कर दी जाएगी ।
िटप्पणी1:- िजस उम्मीदवार के पास पढ़ाई पूरी करने के बाद
माध्यिमक िवद्यालय ूमाण-पऽ हो उसे केवल आयु से
सम्बद्ध ूिविष्ट वाले पृष्ठ की अनुूमािणत/ूमािणत
ूितिलिप भेजनी चािहए ।
िटप्पणी 2:- उम्मीदवार यह ध्यान रखे िक आयोग उम् मीदवार की जन् म
की उसी तारीख को ः वीकार करेगा जो आवेदन-पऽ
ूः तुत करने की तारीख को मैिशकुलेशन/उच् चतर
माध् यिमक परीक्षा ूमाण-पऽ या समकक्ष परीक्षा के
ूमाण पऽ में दजर् है और इसके बाद उसमें पिरवतर्न के
िकसी अनुरोध पर न तो िवचार िकया जाएगा और न ही
उसे ः वीकार िकया जाएगा ।
िटप्पणी 3:- उम्मीदवार यह भी नोट कर लें िक उनके द्वारा िकसी
परीक्षा में ूवेश के िलए जन्म की तारीख एक बार
घोिषत कर देने और आयोग द्वारा उसे अपने अिभलेख में
दजर् कर लेने के बाद उसमें बाद में या बाद की िकसी
परीक्षा में िकसी आधार पर पिरवतर्न करने की अनुमित
नहीं दी जाएगी ।
9. (i) जम्मू एवं कँमीर राज्य के अिधवासी के रूप में आयु में छूट के
लाभ का दावा करने वाले उम्मीदवार को उस िजला मिजःशेट
से, िजसके क्षेऽािधकार में वह सामान्यत: रहा है या जम्मू एवं
कँमीर सरकार द्वारा उसके ःथान पर नािमत िकसी अन्य
ूािधकारी से इस आशय के ूमाण-पऽ की
अनुूमािणत/सत्यािपत ूित ूःतुत करनी होगी िक पहली
जनवरी, 1980 से 31 िदसम्बर, 1989 तक की अविध के दौरान
वह सामान्यत: जम्मू एवं कँमीर राज्य में ही रहा है ।
(ii) आयु में छूट चाहने वाले ऐसे उम् मीदवार को जो रक्षा सेवाओं में
कायर् करते हुए िवकलांग हुआ है, महािनदेशक, पुन: ः थापना,
रक्षा मंऽालय से पिरिशं ट-II में िदए गए फामर् पर इस आशय
का एक ूमाण-पऽ लेकर इसकी एक अनुूमािणत/ूमािणत
ूितिलिप ूः तुत करनी होगी िक वह रक्षा सेवाओं में कायर् करते
हुए, िवदेशी शऽु देश के साथ संघषर् में अथवा अशांितमः त क्षेऽ
में फौजी कायर्वाही के दौरान िवकलांग हुआ और पिरणामः वरूप
िनमुर्क् त हुआ ।
(iii) आयु में छूट का दावा करने वाले भूतपूवर् सैिनक, कमीशंड
अिधकारी और ई.सी.ओ./एस.एस.सी.ओ. सिहत जैसा उनके
अपने मामले में अपेिक्षत है, ूमाण‐पऽ की
अनुूमािणत/ूमािणत ूितिलिप, सम् बद्ध ूािधकािरयों से लेकर
पिरिशं ट-III में िदए गए फामर् में ूः तुत करें ।
(iv) िनयम 5 (ख) के अंतगर्त आयु सीमा में छूट के िलए दावा करने
वाले सरकारी कमर्चािरयों को अपने िवभाग/कायार्लय के अध् यक्ष
से पिरिशं ट-IV में िदए गए फामर् ूमाण–पऽ की मूल ूित
ूः तुत करनी चािहए ।
10. जो उम् मीदवार शारीिरक रूप से अक्षम होने का दावा करता है,
उसे संबंिधत सेवा/पदों पर कायर् िनं पादन हेतु, एक या अिधक
शारीिरक अपेक्षाओं/क्षमताओं जैसा िक इंजीिनयरी सेवा परीक्षा,
2013 की िनयमावली के िनयम 19 में िदया गया है को पूरा
करना होगा । उसे सरकारी अः पताल/िचिकत् सा बोडर् के द्वारा
पिरिशं ट-V में िदए गए ूारूप में जारी ूमाण–पऽ की एक
ूमािणत ूित ूः तुत करनी होगी ।
11. उम् मीदवार यह ध् यान रखें िक इंजीिनयरी सेवा परीक्षा, 2013 में
उनकी उम् मीदवारी पूणर्त: अनिन् तम है तथा उनके िनधार्िरत
पाऽता की शतोर्ं को पूरा करने पर आधािरत है । यिद िलिखत
परीक्षा तथा साक्षात् कार परीक्षण के पहले या बाद में िकसी ः तर
पर जांच करने पर यह पाया गया िक वे पाऽता की िकन् हीं शतोर्ं
को पूरा नहीं करते तो उनकी उम् मीदवारी आयोग द्वारा रद्द कर
दी जाएगी ।
12. उम्मीदवारों को अपने आवेदन-पऽों के साथ िनम्निलिखत ूलेख
अनुलग्न कर आयोग को भेजने हैं :
(i) आयु के ूमाण-पऽ की एक अनुूमािणत/ूमािणत ूितिलिप ।
(ii) शैिक्षक योग्यता के ूमाण-पऽ की एक अनुूमािणत/ूमािणत
ूितिलिप ।
(iii) अनुसूिचत जाित/अनुसूिचत जनजाित/अन्य िपछड़ी ौेिणयों के
होने का दावा करने के समथर्न में ूमाण-पऽ की एक
अनुूमािणत/ूमािणत ूितिलिप । (जहां कहीं लागू हो) ।
(iv) अ.िप.व. के रूप में छूट की अपेक्षा करने वाले उम् मीदवार बीमी
लेयर में नहीं होने के दावे के समथर्न में पिरिशं ट-I के पैरा ख 1
में उिल् लिखत ूारूप में पिरवचन देंगे ।
(v) आयु में छूट के दावे के समथर्न में ूमाण-पऽ की एक
अनुूमािणत/ूमािणत ूितिलिप । (जहां लागू हो) ।
(vi) शारीिरक अक्षमता के डाक् टरी ूमाण-पऽ की एक
अनुूमािणत/ूमािणत ूितिलिप । (जहां लागू हो) ।
उपयुर्क्त ूमाण पऽ मूल रूप में साक्षात्कार के समय ूःतुत
करने होंगे ।
िटप् पणी:-उम्मीदवारों से यह अपेक्षा की जाती है िक वे आवेदन-पऽ के
साथ भेजे जाने वाले ूमाण-पऽों की अनुूमािणत/ूमािणत
ूितिलिपयों पर अपने हःताक्षर करें तथा तारीख भी िलखें ।
13. िजन मामलों में आवेदन-पऽ सब अनुलग्नकों रिहत या कुछ
अनुलग्नकों सिहत ूाप्त होंगे, उनमें उम्मीदवारी रद्द कर दी
जाएगी । उम्मीदवार यिद गायब अनुलग्नकों को बाद में भेजते
हैं तो उन्हें ःवीकार नहीं िकया जाएगा । उम्मीदवार यह अवँय
सुिनिँ चत कर लें िक आवेदन-पऽ ठीक से भरा गया है और
उसके साथ सभी संगत अनुलग्नक भेजे गए हैं । आवेदन-ूपऽ
का कोई भी कॉलम िरक् त न छोड़ा जाए ।
14. उम्मीदवार अपने आवेदन के संबंध में आयोग को जो पऽ भेजें
उनमें परीक्षा का नाम, अपना पूरा नाम, अनुबमांक तथा जन्म
की तारीख अवँय िलखें ।
