How to Apply: The filled-in application, along with the photocopies of relevant documents for their claims, shall be submitted in person or by Registered Post Acknowledgement Due, to the concerned Chairperson of the Department, so as to reach within the last date.
Last date for submission of application without fine: 28.07.2016.
Last date for submission of application with a fine of Rs. 200: 01.08.2016.
